1) Tell us about your background and how Siftwell Analytics was started?

Siftwell was founded in 2022 by former MCO leaders with the goal of creating a tool they knew could provide insights to better inform positive engagement and better health outcomes for the complex, behavioral health-impacted populations they were serving. Siftwell’s mission is to empower health plans to better understand the multifaceted needs of their member population so interventions can be designed and deployed early, with more precision.  A partnership with Siftwell means community health plans can fully understand their members’ health, including their health-related social needs, allowing plans to design responsive care delivery models that empower members to live the healthiest lives they can live.

2) What are the key problems Siftwell Analytics is addressing?

Siftwell combines advanced AI with deep managed care leadership experience, clinical operations workflow know-how, and that leverages our understanding of and experience within the community health plan context. We help our current and prospective partners answer questions that are fundamental to their missions and operations.

We serve a diverse client base, including CO-OPs, Medicaid plans, ACA plans, and the HEDIS, marketing/enrollment, and population health teams inside them. We provide tailored solutions, aiming to enhance healthcare outcomes and reduce costs across various healthcare settings.

Our key areas of focus include:

  • Voluntary disenrollment and retention – Proactively identifying members likely to leave or stay in the plan with over 90% accuracy, enabling tailored retention campaigns and resource allocation strategies.
  • Risk Adjustment – Predicting members’ likelihood to attend Annual Wellness Visits and develop chronic conditions within the next 12 months, aiding clinical teams in ensuring proper documentation and diagnoses for at-risk individuals.
  • Population Health – Anticipating members’ new diagnoses and associated costs over the next year, along with recommended interventions, by analyzing clinical and social determinant datasets, empowering care programs to improve health outcomes.
  • Quality – Identifying members prone to engage in and benefit from quality measure activities like cancer screenings and medication adherence, facilitating targeted interventions to enhance healthcare quality.

3) What have you enjoyed or what do you look forward to about working with Tau Ventures?  

Tau Ventures has been a fantastic partner to us in just the short time we have been working together. In just a few weeks of being a portfolio company, we have been connected with two exclusive in-person event opportunities, several network contacts, and also received guidance related to PR and company news distribution.
